To attend Pet Fashion Week NY 2009, you must provide identification to qualify your business, and separate identification to qualify each individual employee from your company. Please provide ONE form of company identification and ONE form of employee identification.

Company identification includes one of the following:

• The mast page from an approved media outlet with individuals’ names and titles listed
• A copy of or internet link to a current article published in an approved media outlet within the past 6 months

Employee identification includes one of any of the following:

• Imprinted business card
• Business Credit Card
• W-2
• Mast Page
